Objective ADHD testing, explained without the sales pitch

Yes, objective tools exist. Yes, some are FDA-cleared and genuinely useful. No, none of them can diagnose your child — and understanding why tells you exactly what they're for.

What the best evidence actually showed

The strongest trial in this space is the AQUA randomized controlled trial: when clinicians assessing children had QbTest reports available, they reached diagnostic decisions substantially faster — with no measurable loss of accuracy — and excluded ADHD more confidently when it wasn't present. Read that carefully: the demonstrated benefit is a faster, more confident clinical process, not a machine that outperforms clinicians.

That's also exactly why regulators cleared these tools as aids. A 20-minute task in a quiet room measures a slice of attention and movement. It cannot see the thing the diagnosis actually requires: impairment across settings, over months, in a child's real life.

When objective testing earns its cost

  • Ambiguous pictures — home and school reports disagree, or scales sit near thresholds.
  • High-stakes skepticism — a parent (or teen) who doubts the diagnosis and deserves more than "trust me."
  • Treatment verification — repeating the test on medication provides a before/after that rating scales alone can't match.
  • Countering bias — norm-referenced data doesn't know your child's birthday or gender, which blunts the referral biases covered in Essay 01.

Typical cash cost runs a few hundred dollars per administration — modest against the cost of years of imprecisely aimed treatment.

Red flag to watch for: any clinic that presents an objective test result — by itself — as the diagnosis, in either direction. That's not what the tools are cleared for, and it's not what the evidence supports.
